Two people were hospitalized with serious injuries after a helicopter accidentally fired on bystanders at the Zapad 2017 military exercises, the online news portal 66.ru cited a source as saying on Tuesday.

And they include include functions tests that are supposed to find these issues. You also do live fire exercises which are on carefully controlled ranges where everything is done in a safe manner. For instance, we found a problem in our m-249 SAW machine guns which caused the bolt to slam forward and fire with the safety on when you close the feed tray under certain circumstances. This error was found because we did an exhaustive test on the weapon, and we were able to recreate the issue.
Considering you live or die by your equipment, any military worth its salt checks and rechecks their shit on a daily basis.
